- Fill out the "Referral form" on this webpage
- We send your friend or family member 1 email telling them about how our services can help them (no hassle, we promise!)
- If and hopefully when the person you referred becomes a patient, we will send a $10 gift card to you in the mail
Really it's that simple!
- You must be a current or former patient of Fit For Play Physical Therapy to be eligible to receive a gift card for a referral
- You only receive a gift card if the person(s) you refer actually become a patient of Fit For Play physical therapy (must complete an initial evaluation)

We can send you a few Care To Share cards that you can personally give your friends and family members. You just put your name on the signature panel, give the card to someone, and make sure they bring it in within their first few visits. Presto... you receive a $10 gift card in the mail.
